EdgeMax + UniFi AP + Multiple VLANs

I just purchased a ERPOE-5 and am having issues setting it up.

Here is what I'm trying to do:

1. Use eth0 for Internet

2. Use eth1 for my primary 192.168.4.0/24 network

3. Use eth2 for secondary subnets 192.168.8.1/24 and 192.168.100.1/24 that currently have no clients (future use)

4. Use eth4 for my AP which will have an IP of 192.168.4.59

I'm primarily confused on how I need to setup my AP. I need to plug it into eth4 because the router offers PoE but its unclear to me how to configure the port. My DHCP server is running on a server with an IP of 192.168.4.253.

I'm not set on this architecture below. If anyone has any suggestions or can help me I would appreicate it.
